Vertical strips technique: With the patient sitting up, begin palpating in a spiral motion at the medial portion of the chest wall and end just below the posterior aspect of breast tissue. Using the dominant hand, palpate downward and then upward without lifting the fingers from the breast. For larger breasts, place the nondominant hand under the breasts to hold it up and enhance ability to palpate effectively.
Circular technique: With the patient lying flat, ask her to raise the arm of the breast being examined behind the head to stretch the breast tissue against the chest wall. Starting at the nipple, palpate the breast using spiral motions, moving in circular pattern toward the periphery until all breast tissue is examined, including the tail.
Wedge or “spokes” of a wheel technique: With the patient in a lying or sitting position, the breast is separated into quadrants resembling the spokes in a wheel or wedge, with the nipple at the center of the “wheel.” Starting at the nipple, palpate outward toward the periphery of the wedge (section of the breast) that is being examined. Using the spiral motion with the pads of the fingers, palpate a few centimeters at a time in each wedge until the entire section is palpated. Move to the next wedge after the entire breast, its tail, and its outer edges have been palpated.
